CANCELLATION POLICY:

Deposit and Rental Fees are FULLY REFUNDABLE if cancellation is made before 30 days of reservation date.

Rental Fees are NOT refundable if cancellation is made less than 30 days prior to reservation date.
Deposit Fees WILL be refunded if cancellation is made less than 30 days prior to reservation date.

Clean Up Responsibilities

**CLIENT IS RESPONSIBLE TO BRING ALL CLEANING SUPPLIES WITH THEM**
Please make sure all the following are completed before you leave…
• Have you swept and mopped the TILE floors?
• Vacuumed the CARPETED floors?
• All signs and decorations removed?
• Have you wiped down all tables and cleaned any debris/stains from fabric chairs?
• Wiped down and cleaned all counters – Kitchen, serving, bathroom?
• Appliances emptied and wiped down inside and out?
• Restrooms swept and cleaned; floors mopped?
• Parking lot cleaned of trash?
• All garbage/trash taken from the facility to the dumpster, including kitchen and bathrooms?
• Turn off all lights and secure the building before you leave, making sure all doors are locked?

CLIENT is responsible for any damages and missing articles and will be charged accordingly

**If we clean up after you, your deposit WILL BE forfeited, and you will also be responsible for any additional charges**
The City of Madisonville is not responsible for any items left behind by you or your guests, so make sure you have removed everything before you lock the door.

Smoking

The City Facilities are non-smoking facilities. No smoking is allowed inside the facilities. If smoking occurs on premises, deposit will be forfeited.

in Case Of Emergency

Fire:
1. Calmly evacuate facility
2. Call 911
a. Give Facility Location:
Kimbro Center, 111 West Trinity Street, Madisonville
3. If the fire is small, extinguish with the nearest fire extinguisher
4. To operate fire extinguisher, follow, P.A.S.S.
a. Pull trigger pin
b. Aim nozzle at the base of the fire
c. Squeeze trigger
d. Sweep from side to side
Tornado:
Tornado Watch means that conditions are favorable for the formation of a tornado, but none have been signed in the area – prepare to take shelter.
Tornado Warning means that a tornado has been sighted in the area **TAKE COVER*
Power Outage:
1. Emergency lights will come on
2. Slowly move occupants toward exits, leave and secure building
3. Call 936-348-2748 to notify facility manager
Heart Attack or Serious Injury:
1. Call 911
a. Give Facility Location
Kimbro Center, 111 West Trinity Street, Madisonville
2. Solicit medical help within the building (CPR. First aid trained persons)

Indemnification

CLIENT agrees to defend, indemnify and hold harmless The City of Madisonville and its employees from and against any and all claims, demands, causes of action or liabilities, arising from CLIENT’s acts or omissions under this Agreement, except as may arise from the negligence or willful misconduct of The City of Madisonville or its employees. The City of Madisonville will not be held responsible for any losses, damages, or injuries. This refers to any loss, damage or injury to persons or possessions that may occur at any function held at the Facility, from any cause, whatsoever, prior to, during, or subsequent to the period covered by this contract. CLIENT will be responsible for the control and supervision of the people in attendance during the use of the facility to ensure no harm is done to persons or property. CLIENT agrees to abide by this Agreement and acknowledges having received a copy thereof. CLIENT will be held financially responsible for any damage to the Facility or equipment, which occurs through the CLIENT’s meeting or event at the Facility.
